UNILAG ISSUES IMPORTANT NOTICE TO ALL NEWLY ADMITTED DISTANCE LEARNING INSTITUTE [DLI] STUDENTS FOR 2023/2024 ACADEMIC SESSION

This is to inform all the newly admitted students that the daily screening exercise is currently ongoing

 

 

 

 

Time: 9.00 am – 3:00 pm daily

 

 

 

 

Venue: Distance Learning Institute, 2nd Floor DLI Administrative Building.

 

 

 

Required documents needed for the screening exercise are as listed below:

 

 

 

 

Application Form (Coloured).

Testimonial (Primary or Secondary) (Original).

O’ Level Certificate(s) (Original).

All Higher Degree Certificates for Direct Entry (e.g. HND, OND, NCE, ICAN and other Professional certificates).

NYSC Certificate/NYSC Exclusion or Exemption Certificate for Direct Entry.

Birth Certificate/Sworn Affidavit that Predates the Admissions Advertisement.

Marriage Certificate, Affidavit for Change of Name, and Newspaper Classification for Married female only.

 

 

 

 

Required documents needed for the registration exercise (after the payment of required fees) are as listed below:

 

 

 

 

Application Form (Coloured).

Admission Letter (Coloured and Laminated).

Testimonial (Primary or Secondary) (photocopy).

Student Information Form (coloured).

O’ Level Certificate(s) (photocopy).

All Higher Degree Certificates for Direct Entry (e.g. HND, OND, NCE, ICAN and other Professional certificates).

NYSC Certificate/NYSC Exclusion or Exemption Certificate for Direct Entry.

Online Generated Letter of Acceptance (Coloured).

Online Generated Biodata Form (Coloured).

Online Course Registration Form Approved by Course Adviser (Coloured).

Letter of Preparedness to Pay All Service Charge (Hand written and signed by applicant).

A Reference Letter from Employer or Sponsor (Original).

A Reference Letter from any of, a Lawyer, Clergy, Imam or Senior Civil Servant

Online generated Student Payment Receipt (Coloured).

Marriage Certificate, Affidavit for Change of Name, and Newspaper Classification for Married (female only).

Birth Certificate/Sworn Affidavit that Predates the Admissions Advertisement.

Health Service Questionnaire (Coloured)

Photographs – passport size with white background (4 copies with name and matriculation number on the reverse side).

Please note that Public Administration and Business Administration admitted students can also come for screening on Saturday 16th and 23rd of March 2024.

 

 

 

Please note that failure to comply and appear for the screening exercise within one month may lead to forfeiture of admission.
